There is no fixed number, but the useful threshold is the point where one negative review cannot move your average or dominate the recent set. For most practices that means a few dozen reviews and a steady flow of several each month, because recency matters as much as total volume.

Two numbers, not one

Total volume determines how much a single review moves the average. Recency determines whether the profile reads as a current picture or a historical one.

A practice with 300 reviews and nothing in eighteen months looks abandoned. A practice with 40 reviews and four this month looks busy. The second converts better, which surprises people who focus only on the total.

The maths of a single bad review

  • With 10 reviews, one 1-star drops a 5.0 to roughly 4.6. Visible and damaging.
  • With 50 reviews, the same review moves it to roughly 4.92. Barely perceptible.
  • With 150 reviews, it is statistical noise.
  • The same review, in every case. What changed is what surrounds it.

How to build the flow without breaking the rules

Ask every patient, not the ones you expect to be happy. Selective solicitation is review gating and is a violation on every major platform.

Make it easy: a direct link, sent at the right moment, once. Do not incentivise, do not screen, and do not route unhappy patients to a private form while sending happy ones to Google. That practice is specifically prohibited and is straightforward to detect.
